Gallica / Provins.  
Bloom:
Purple - pink.  Large, double (17-25 petals) bloom form.  Once-blooming spring or summer.  
Growing:
USDA zone 4b through 8b.  Remove old canes and dead or diseased wood..  Prune after flowering is finished.
